Hormone & metabolic medicine · St. Louis

Your doctor makes nothing on your prescription.

Concierge care. Pharmacy prices.

I am Dr. Scott Harshman. Harshman MD is my solo practice for testosterone and hormone therapy and medical weight management. One membership covers my time and judgment. Your medications and labs pass through to you at exactly what I pay, with no markup — so the advice you get has nothing riding on it.

The whole bill

Most clinics profit twice. I profit once.

A clinic that marks up your testosterone has a reason to keep you on it. A clinic that marks up your labs has a reason to draw more. I took that incentive off the table: medications and labs are billed to you at the price I pay, documented, with nothing added.

Here is a full first year, itemized. Pick the one that looks like you.

Under $250 a month, with medication and labs included. Testosterone is priced on my usual protocol — 200 mg weekly for the first six weeks, then titrated to 100–150 mg on your labs, which works out to four $35 vials across the year.

What the pharmacy charges

These are the pharmacy's prices, not mine, and they are what you pay. A vial lasts as long as your dose makes it last, so here is what each one costs and roughly how far it goes at a standard dose.

10 weeks at the 200 mg starting dose, 13–20 weeks once titrated

Once your dose is settled, creams and capsules are dispensed in the largest sensible quantity — a 90 g jar of estrogen cream costs $10 more than a 30 g one and lasts three times as long, and there is no reason for me to keep selling you the small size. Durations assume standard dosing and will shift with yours — a higher weekly dose means the same vial covers fewer weeks. Laboratory work is billed separately at cost. Testosterone therapy runs on a set monitoring schedule; otherwise you have a baseline panel and then labs only when something needs checking. The pharmacy and the lab set these prices and can change them; I pass through whatever they invoice.

What I treat

A narrow practice, on purpose. Three areas I can follow closely for a small panel, for men and women.

Men

Testosterone therapy for men

Evaluation and management of low testosterone. Most men start at 200 mg weekly for six weeks, then titrate down to 100–150 mg based on what the labs show — the dose follows the bloodwork rather than sitting where it was set at intake. Most men need nothing beyond testosterone itself; where an adjunct like HCG is the right call, it is included.

Women

Hormone therapy for women

Menopausal hormone therapy built on a topical estrogen cream and oral progesterone, with low-dose testosterone added only when your symptoms call for it. The first two or three months go to finding your dose, working from small jars so nothing is wasted while we adjust; after that the regimen settles and the cost drops. Directed by how you feel rather than a fixed protocol. You have a baseline panel at the start and then labs only when there is a reason for them — and if you are on testosterone, a short recurring panel that looks at your testosterone level, estradiol, and hemoglobin and hematocrit.

Men & women

Metabolic and weight management

Medical weight management using injectable GLP-1 therapy where it is appropriate. Most people start at the lowest dose for two to four weeks, step up once, and hold there — going higher only if you ask and it is the right call medically, rather than climbing the ladder by default. Paired with the nutrition and training work that makes the result hold once the medication stops.

Physician-led, in practice

Not a slogan. It shows up in how the practice runs.

The physician who reads your labs is the one prescribing your care. There is no mid-level layer and no rotating staff.

Your medications come from one licensed compounding pharmacy I have vetted, not a research-chemical vendor.

Doses are titrated to your labs and your symptoms over time, not set once at intake and forgotten.

The panel is capped. When it is full I hold a waitlist rather than dilute the care — that is the trade I would want as a patient, so it is the one I offer.

Request a consult

Tell me what you are trying to solve and we will find a time to talk. There is no charge for the first conversation. New patients are accepted as panel space allows.
